How are appointments scheduled?
Please call me directly at 773-844-9668.  If I am not available, please leave a message on my voice mail with your name, telephone number, and a good time to call you.  You may also call my assistant, Barb Svarz at 312-578-1741, to set up an  initial appointment.  It’s helpful for me to have some history as to why you want to be evaluated, thus making sure that I would be the right person to treat you.


What are your appointment hours?
I see patients on Mondays and Thursdays from 8:00am – 7:00pm.


What is your cancellation policy?
I do require 24 hours of advance notice to cancel a session without charge.  If I have not received the required 24 hour notice of cancellation,  I do bill in full for the session.  However, if given ample notice, I am typically able to reschedule your appointment easily.


Do you take insurance?
I am an out of network physician and am not on any insurance panels.  I do not bill claims to insurance and have opted-out of Medicare.  Payment arrangements are exclusively between myself and my patients.  Patients do receive an itemized receipt that can be submitted to insurance (but not Medicare).
